VICE PRESIDENT - HOME CARE
SERVICES
The Vice President - Home
Care Services will be responsible for the overall Long Term Home Health Care
Program (LTHHCP) -- operational, fiscal and clinical leadership of Home Care
Services of BSNY. The incumbent will ensure that service delivery standards
are being maintained; promote the agency within the local market; strengthen
the home health care delivery model to optimize growth; drive quality
standards; and uphold regulatory compliance.
Minimum Requirements:
-
BSN and Masters in Nursing, Heath Care Administration or related
area.
-
Current and valid New York State RN license, or the ability to
acquire a valid NY license through reciprocity.
- At least 10
years progressive Home Health Care experience, with a minimum of 5
years in a progressive leadership/management role (s).
- Experience and
knowledge of JCAHO and NYSDOH home care requirements with demonstrated
and significant participation in the survey process.
- A demonstrated
history of growing volume, gaining market share, and improving bottom
line performance and an understanding of marketing principles.
- Ability to
cultivate professional relationships to ensure a solid referral base.
- Ability to
understand and use technology to create and sustain competitive
advantage.

Must be
knowledgeable in the principles of quality improvement, case management,
and skilled/assisted/independent living and the establishment
of best practices in these areas.

FULL
TIME SUPERINTENDENT, LIVE IN APARTMENT
High School Diploma
or GED required plus 3 years previous experience. Valid Low-Pressure Boiler
License, Certificate of Fitness for Standpipe and Sprinklers required.
Valid NYS Driver’s License, CDL preferred. Required to reside on-site and
respond to all emergencies as needed. Has overall responsibility for
building maintenance. Insures all work orders are completed timely and
efficiently.
COMMUNITY HEALTH NURSE, HOME HEALTH CARE
The Community Health
Nurse provides professional nursing care and prescribes treatments for an
assigned population of geriatric and adult patients in their homes, as
prescribed by the physician and plan of care. Coordinates comprehensive
nursing services with patients and families, and arranges for other agency
services necessary to meet patient’s needs. Home care experience preferred.
Minimum one year acute care experience required. Current/valid RN or LPN NY
State license. BSN preferred. Current NYS Driver License. Bilingual a plus.
REGISTERED NURSE [RN]
Responsible for the
care of a designated group of residents. Utilizes all components of the
nursing process in providing care that reflects an understanding of
individual differences and needs, and recognition or residents' rights.
Directs and assists assigned nursing personnel in resident care. BSN
preferred. Current/valid NYS Registered Nurse License. One to three years
nursing experience preferably in a long care setting. Experience in caring
for dementia patients highly desirable.

CLINICAL DIETITIANS, FOOD & NUTRITION
Provides nutrition
therapy to residents. This will include assessment and evaluation of
residents' nutritional needs, planning and implementing nutritional care
services for residents. Documents care provided in residents' medical
records. Upholds professional standards of performance, departmental
policies, local, state, and federal regulations. Supervises dietary workers.
BS in Food and Nutrition Required. Registered Dietitian Preferred. Current
ADA membership required. Minimum one year clinical and supervisory
experience in a health care environment.
OFFICE
ASSISTANT, SOCIAL SERVICES
Responsible to handle
admissions, discharges, Medicare and hospital letters, arrange
transportation for residents, schedule IDCP meetings, keep statistics.
Organized, flexible and good interpersonal skills required. Able to handle
large volume of work. Knowledge of computers and office software packages
(Word, Excel, etc.) High School Diploma or GED required.
OPERATIONS MANAGER, FACILITIES
The Operations
Manager of the Facilities Department shall plan, organize, supervise and
control the daily activities of the Plant Operations functions. Minimum of
five years experience in progressively responsible administrative positions
in a health care environment. Stationary Engineer or Refrigeration, H.V.A.C.,
Boiler Operator licensure preferred to demonstrate proficiency in plant
operations. Sprinkler and standpipe license also required. Bachelor's
Degree preferred.
FIREMAN,
FACILITIES
Operates and repairs
boilers, heating systems, piping and general maintenance throughout the
facility. HS diploma or GED required. Certificate of fitness for operation
of boilers required.
GROUNDSKEEPER
Full
time, 7:00am - 3:00pm. Utilizing hand and powered equipment, maintains
exterior grounds, including lawns, flower beds, shrubbery, roadway and
sidewalks. Maintains equipment. Removes snow and ice. Other duties as
assigned. High School diploma required.
RECREATION THERAPIST—DAYS/EVENINGS/WEEKENDS
Conducts diversified
programs and activities appropriate to the interest,
physical and mental capabilities of all residents. Participates in the
program- and care-planning processes. Therapeutic Recreation
experience required. Demonstrated skills in one of the following
areas: music (voice or instrumental), arts and crafts, movement/exercise,
painting, ceramics. BA/BS in Recreation Therapy/related field, with a
minimum of three years experience in dementia care. CTRS certification
required with long term care experience, i.e. MDS completion, individualized
care plans, initial assessments and quarterly progress notes. Must be
able to think outside the box in terms of creating diversional and
goal-oriented programs for residents with dementia.
