SOCIAL WORKERS/LMSW
We are seeking an individual to provide social work services to patients and
families in their homes as part of our Long Term Care Program. Requires
Master’s degree in Social Work, 2-3 years of home care experience, NYS
licensure and a valid NYS driver’s license. Competitive salary and

COMMUNITY HEALTH NURSE, HOME HEALTH CARE
The Community Health
Nurse provides professional nursing care and prescribes treatments for an
assigned population of geriatric and adult patients in their homes, as
prescribed by the physician and plan of care. Coordinates comprehensive
nursing services with patients and families, and arranges for other agency
services necessary to meet patient’s needs. Home care experience preferred.
Minimum one year acute care experience required. Current/valid RN or LPN NY
State license. BSN preferred. Current NYS drivers license. Bilingual a plus.
REGISTERED NURSE [RN]
Responsible for the
care of a designated group of residents. Utilizes all components of the
nursing process in providing care that reflects an understanding of
individual differences and needs, and recognition or residents' rights.
Directs and assists assigned nursing personnel in resident care. BSN
preferred. Current/valid NYS Registered Nurse License. One to three years
nursing experience preferably in a long term care setting. Experience in caring
for dementia patients highly desirable.
CLINICAL NURSE MANAGER,
7:00 am - 3:00 pm
Responsible for the
administration and coordination of resident care on a 24 hours basis on the
assigned unit. Manages staff activities and behavior. Works collaboratively
with the program director, physicians and other disciplines in the
development, implementation and direction of new healthcare delivery models.
Collaborates with the supervisors on evenings and nights and resident
service coordinators. Graduate of an accredited School of Nursing. BSN
preferred. Current valid NYS Registered Professional Nurse Licensure. 1 - 3
years Long Term Care Nursing.
PHYSICAL THERAPIST,
HOME HEALTH CARE, PER DIEM, DAYS
Provides therapy services that meet
standard physical therapy practices,
upon a physician's referral, to patients
in the home setting. Interacts with adult and geriatric populations.
Bachelor's degree in Physical Therapy. New York State Physical Therapist
license required. Home Care geriatric experience required.

Responsible for
administering routine services to meet the nursing needs of residents and
assist in direct resident care activities. Valid NYS Practical Nurse License
required. Ability to communicate effectively. One year med/surg experience
preferred.
SENIOR
ACCOUNTANT
Under the supervision
of the Controller, the Sr. Accountant is responsible as a backup for bank
reconciliation, preparing monthly JE, performing analysis for Balance Sheet
and Income Statement accounts, performing A/P and A/R reconciliation, assist
in preparation of annual cost reports, assist in monthly and YE closing,
assist in preparing fiscal YE ARP report, preparation of Quarterly payroll
tax reconciliation, YE W-2 reconciliation, assist in preparation of YE tax
return and other assignments. BS in Accounting required. Knowledge of
computers, accounting software packages, Excel and ability to adapt to
changing priorities. Minimum 2 years experience required in related field,
preferably in a health care environment.
