The Palliative Care
Dementia Consortium is committed to developing best practices in palliative
care for all nursing home residents with dementia and is a resource for
anyone needing information or assistance regarding palliative care. The
consortium consists of Schervier Nursing Care Center, Ravazzin Center for
Social Work Research on Aging, Andrus on Hudson, Cabrini Center for Nursing
and Rehabilitation, Kings Harbor Multicare Center, and Morningside House
Nursing Home.

The goal of this
manual is to provide education materials regarding palliative care for
nursing home administrators, management, trainers, and staff. It uses a
“Train-the-Trainer” format. The materials are presented in a ready-to-use,
easily accessible, compact form. The manual is divided into seven training
modules: Understanding Dementia; Palliative Care; Culture, Spirituality, and
Religion; Advance Directives; Pain and Pain Management; Teamwork and Care
Planning; and Creative
